Role & Responsibilities
As Sollis Health expands, we are looking to bring on an experienced Telehealth PA or NP to build and provide telehealth medicine to our patients. Responsibilities include:

  • Managing all inbound telehealth needs and appropriately triaging calls that might require in-person medical visits
  • Spearheading our aftercare/follow-up process for all patients seen in our offices, at home, or via telemedicine
  • Managing any on-going clinical care events (hospital admissions and extended home care cases)
  • Coordinating patient’s homecare needs and communicating with HHAs, nurses and case managers
  • Effectively documenting follow-up notes and patient feedback
  • Identifying patient specialist referral requests and communicating patient preferences with Care Navigator
  • Performing new member clinical welcome calls to answer any initial questions that members might have about Sollis Health
  • Providing clinical home visits when necessary


  • 2-10 years experience as a Physician Assistant/Nurse Practitioner with background working in family medicine, urgent care or emergency medicine
  • Experience treating both adult and pediatric patients
  • Comfortable working with an EMR (we use eClinical Works)
  • Comfortable working with a CRM (we use Salesforce)
  • Strong history of creating excellent customer experiences
  • Experience operating telehealth platforms a plus

  • Strong clinical skills as either a PA or NP
  • Strong process and organizational skills
  • Excellent attention to detail
  • Comfort with online systems and ways to improve them
  • Persistence and follow-up skills
  • A warm and welcoming personality
  • Comfort working in teams and triaging medical needs
  • Excellent social, interpersonal, and communication skills
  • Must be friendly and easy to talk to – both on the phone and in person, with patients and staff
